Safety briefing record template: Word and PDF
A briefing record documents who received instruction, on which topics and when. Download the editable Word file or PDF without signing up. Use it following instruction that actually took place and matched the work; a signed list cannot replace the content or delivery. The requirements described here concern Germany.

Before the briefing: define the occasion and content
Select content from the actual task, risk assessment and relevant operating instructions. “General safety” is usually too vague to show what was covered later. Identify the work area, hazards discussed and practical rules explained.
Establish language needs, prior knowledge and opportunities for questions beforehand. Where materials are translated or explained orally, record that as appropriate. The template provides record fields, not a ready-made lesson suitable for every item of equipment.
Information that makes the record meaningful
Record the actual date, location and instructor. Identify materials used by title and revision so readers can establish whether the briefing relied on an earlier operating instruction or a work method that has since changed.
- Project or business, work area and reason for the instruction.
- Date, time where useful, instructor and content covered.
- Names of people actually instructed and their company where needed.
- Materials used, language, questions discussed and any practical exercises.
- Required acknowledgement, outstanding follow-up instruction and responsible contact.
Distinguish delivery from acknowledgement
Explain the content clearly, allow questions and, where needed, ask people to explain or demonstrate important actions. Record only attendance that actually occurred. Arrange a separate session for absentees rather than adding them as if they had attended.
Signature fields can document acknowledgement. Whether and which signatures are required depends on the applicable rules. Section 12 BetrSichV expressly requires a written record of the date and names; that provision alone does not establish a general signature requirement for every topic. Specific rules may impose additional requirements.
Fictional example: a changed site entrance
Entirely fictional record example: at the “Musterhof” project, three workers are briefed before starting work on 13 October 2026 about the changed pedestrian route, separation from deliveries and new assembly point. The record identifies the site plan dated 12 October 2026, the routes explained and an outstanding question about the late shift, with somebody assigned to clarify it.
The example illustrates record keeping, not the completeness of required instruction. Equipment-specific content and the actual workplace conditions must also be considered.
Repeat instruction and record retention
Section 12 ArbSchG requires instruction on relevant occasions and, where necessary, at regular intervals. For work equipment, section 12 BetrSichV requires instruction before use and at least annually thereafter. An annual schedule does not remove the need for instruction when relevant changes occur. Set dates from the task and applicable rules.
Check names, content revision and actual attendance before filing. Restrict access to the intended recipients. The template does not prescribe a universal retention period; determine it from the record’s purpose and applicable requirements.
